flotice of Violation Based on the results of the NRC inspection conducted on January 11, 1979, certain activities appear to be in noncompliance with NRC regulations a.id the conditions of your NRC License No. 35-15112-01 as indicated below:
1.
License Condition 17 requires that licensed material be used in accordance with statements, representations, and procedures contained in the license application dated May 1,1977.
Contrary to this requirement, annual refresher training was not administered to radiographers or radiographers' assistants during the period September-1977 to December 1978 as specified in Section 5.2.1 of the licensee's Radiation Safety Manual of Administrative Procedures.
This is an infraction.
2.
10 CFR 34.22 requires in part that each radiographic exposure device shall be provided sdth a lock designed to prevent unauthorized or accidental removal or exposure of a sealed source and shall be kept locked at all times except when under the direct surveillance of a radiographer or radiographers' assistant.
Contrary to this requirement, two exposure devices were stored with keys inserted in their locks nn the day of the inspection.
This is an infraction.
3.
10 CFR 20.203(f) requires in part that each container of licensed material shall bear a label with the radiation caution symbol und the words, " CAUTION, RADIOACTIVE MATERIAL" or " DANGER, RADI0 ACTIVE MATERIAL".
Contrary to this requirement, one exposure device containing licensed material was not so labeled on the day of the inspection.
This is a deficiency.
