Facility Closeout Survey for 76 Treble Cove Rd and 700 Summer St. facilities

Dear Mr. Naquin,

Passport Systems, Inc. requests the termination of license Special Nuclear Material (SNM)-2016 Docket No. 70-7022 and submits the following information in support of this request

1. The completed and signed NRC Form 314 U.S. NRC Certificate of Disposition of Materials. Enclosed as.
2. All materials authorized under license SNM-2016 have been physically transferred to Sensor Concepts &

Applications, Inc. (SCA) in Glen Arm, MD for possession and use under the license SNM-2017-Docket No.

70-7020. The completed U.S. NRC Nuclear Material Transaction Form 741 reports are included as the records of transfer and acknowledgement of receipt from SCA in Attachment 2.

3. All radioactive materials that were possessed under the License SNM-2016 were sealed sources and were leak tested at six month intervals. While in our possession, no source was ever found to have an external contamination level greater than 0.005 µCi. Therefore, there was no concern for contamination at the facilities authorized under license SNM-2016. The latest leak tests are enclosed as Attachment 3.
4. Final inspection and radiation surveys were performed at 76 Treble Cove Rd, N. Billerica facility prior to releasing to the landlord, and 700 Summer St. S. Boston, Conley Terminal. These inspection and radiation surveys confirmed that no sources of radiation remained at either facility. The surveys did not result in any measurements that exceeded background radiation levels at either facility. The final inspection and survey reports are enclosed as Attachment 4.
