This facsimile confirms the conversation between Mr. J. M. Brown and Mr.

B. W. Riley, of your office, at 1530 on November 15, 1979.

On October 19, 1979, the RCIC turbine failed to start due to a sheared bolt on the steam exhaust check valve F040 (LER 1-79-74). The unit resu=ed power operation with the RCIC system under an LCO condition while awaiting parts to repair the check valve. On November 15, 1979, the valve was repaired and a leak rate test was performed.

The test results exceeded that allowed by Technical Specifications 3.6.1.2b.

An investigation revealed that the broken bolt with its accompanying nut had become lodged in the seat of RCIC steam exhaust isolation valve F001. The bolt and nut were removed, the seat and disk inspected for damage (none found), and the leak rate test completed satisfactorily.

This event is reported under Technical Specifications 6.9.1.8b.
