Appendix A NOTICE OF VIOLATION Michigan Testing Engineers, Inc.
License No. 21-14810-01 Based on the inspection conducted on October 24, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 and 2 are deficiencies.
1.
10 CFR 20.401(b) requires that you maintain records showing the re-sults of monitoring required by 10 CFR 20.205(c), " Procedures for picking up, receiving, and opening packages."
Contrary to this requirement, as of the day of inspection, you have failed to maintain records of the monitoring you performed of in-coming packages containing more than 100 curies of iridium-192.
2.
10 CFR 34.26 requires that you conduct a quarterly inventory to account for all sealed sources, to maintain records of the inventories for inspection by the Commission, and that these records shall include the quantities and kinds of byproduct material, location of sealed sources, and the date of the inventory.
Contrary to this requirement, as of the day o inspection, you failed to maintain complete records of the quarterly inventories that you made in 1979, in that all of the required information is not contaited on your quarterly inventory form.
