Page 1 of 1 Docket No. 50-338 Report No. LER 79-074/03L-0 Description of Event On 5-16-79, at steady state operation, during the performance of 1-PT-31.7.2, the pressurizer level indicator L-1460 failed high. This is contrary to T.S. 3.3.1.1. Action Statement #7 of the Tech. Spec. was entered and the transmitter was left in the tripped condition. This event is reportable as per T.S. 6.9.1.9.b. Probable Consequences of Occurrence , The operability of the protective instrumentation systems and interlocks ensure that 1), the associated reactor trip will be initiated when the parameter monitored by each channel or combination thereof exceeds its setpoint, 2) the specified coincidence logic is maintained, 3) sufficient redundancy is maintained to permit a channel to be out of service for testing or maintenance, and 4) suf-ficient system functional capability is available for protective and ESF purposes from diverse parameters. Since the level indicator was in the trip mode when it failed high and the minimum number of channels were operable, the safe operation of the plant was not affected. At no time was the safety and health of the public endangered. There are no generic implications for this item. Cause of Event Since the transmitter was operable before the performance of the test and failed while in the test position, the cause of the event is unknown at this time. Immediate Corrective Action A functional test was performed and passed on the transmitter. The instru-ment wires were reconnected and the transmitter returned to operability. Scheduled Corrective Action Surveillance of the transmitter was continued and the transmitter continued to perform correctly. Actions Taken To Prevent Recurrences No further action taken at this time. 2230 35i
