On May 4,1979, while investigating a simultaneous Hi and to flow alarm in the sample line for the containment particulate monitor, Itt-RMS-159, the particulate filter drive mechanism was found inoperable due to a broken slow speed motor shaft.

Probable Consequences of Occurrence:

Channel PJ!-10tS-159 was declared inoperable. The required grab samples were taken. In addition, the containment atmosphere gaseous radioactivity moni-toring system and containment sump discharge flow measurement systems were cperable. The health and safety of the public were not endangered. There are no generic implications.

Causes of Occurrence:

The slow speed motor shaft for the filter paper on Pli-RMS-159 severed pre-venting the f11ter paper from traveling.

Immediate Corrective Action:

A maintenance report was submitted to the instrument shop to determine why RM-RMS-159 was inoperabic. The slow speed motor shaf t and filter paper were replaced. Once the work was completed, the channel was returned to service.

During this period appropriate grab samples were taken.

Scheduled Corrective Action:

None.
