APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License No. CPPR-162 Based on the NRC inspection February 5-8, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Criterion III of Appendix B to 10 CFR 50, as imple-mented by the PSAR Section 17.1A.3.4, design changes, including field-initiated changes are to be controlled by written procedures which specifically apply to changes.... Construction Engineering Procedure (CEP) 3.01 establishes the methods and assigns responsi-bilities for preparation, processing and control of Field Change Requests for design changes to approved designated design agency drawings.
Contrary to the above, reactor containment concrete base mat place-ment No. R1-AM01-1B was completed prior to a Field Change Request being initiated to obtain design agency approval to use 1-1/2 inch aggregate in lieu of 3 inch aggregate which was specified on C. F.
Braun's approved drawing No. S-009, Revision 5.
Additionally, no C. F. Braun Engineering Change Notice to this drawing existed to permit this aggregate substitution.
This is an infraction.
