Dear Sir:
On February 20, 1979, a report was made under the provisions of 10CFR50.55(e) concerning the inoperability of certain AC and DC Solenoid Operated Valves which upon inspection were found to have parts missing.
In accordance with the reporting requirements of 10CFR21, the following information is submitted:

Des,cription of defect, deficiency, or failure to comply:
All 125 volt coils in DC Solenoid Operated Valves were replaced in the field with high temperature coils capable of sustained operation at control circuit voltages up to 140 Volts DC.
Although Field Quality Control was required to witness the coil replacements on all class 1E valves, certain parts were inadvertently emitted when re-assembling some of the 50V's.

2 During Hot Functional testing, several DC Solenoid Operated Valves failed to operate.
Upon inspection it was determined that certain parts in the 50V's were missing, presumably as a result of omissions that occurred when the coils were re-placed.

After discovering that parts were missing from some DC S0V's, a spot check was performed on selected AC SOV's.
Several AC S0V's were found to be missing parts; this can also be attributed to omissions during reassembly, as all AC S0V's with general purpose enclosures must be partially disassembled in order to connect conduit and cable to the SOV.

Corrective action which has been, is -being or will be taken, the indi-vidual responsible and the length of time to complete the action:
In order to determine if any of the 50V's are lacking parts, Stone & Webster Field Quality Control and Advisory Operations will inspect all DC S0V's which had their coils replaced with~
the.high temperature DC coil.
DC SOV's found to have parts missing will be reassembled with all parts replaced in accord-ance with appropriate manufacturer's installation and mainten-ance instructions.
All 120 Vo.it AC Solenoid Operated Valves with general purpose enclosures will be inspected for missing parts by Stone and Webster Field Quality Control and Advisory Operations.
All AC SOV's with threaded conduit hubs, explosion - proof enclosures, or water proof enclosures will be tested to insure operability.
' Any AC Solenoid Operated Valve found to be missing parts wi.ll be reassembled with all parts replaced in accordance with appropriate manufacturer's installation and maintenance in-structions.
All work on both AC and DC Solenoid Operated Valves will be completed prior to fuel loading.
