Dear Sir:
On January 17, 1979, a report was made under the provisions of 10CFR50.55(e) concerning a defective end casting on one of the Recirculation Spray Pump fttors. The defect permitted bearing grease leakage causing gross failure of the motor lower bearing.
In accordance with the reporting requirements of 10CFR21, the following information is submitted:

North Anna Power Station, Unit 2 Recirculation Spray Pump Motor 2-RS-PM-1B C.

Description of defect, deficiency, or failure to comply:
A defect in the lower end casting of Recirculatirsn Spray Pump Motor 2-RS-PM-1B allowed grease to escape from the lower bearing during a test run of the motor.
The loss ~ of bearing lubricant caused gross failure of the lower bearing with resultant damage to the motor shaft, the upper guide bearing, and the upper and lower seal rings.
The problem was evidenced by noise and smoke emanating from the motor in the ninth hour of operation.

2 operation was terminated, and an examination revealed that bearing grease had leaked from a shrinkage cavity in the area of the grease relief plug in the lower end casting.

Similar components, activities, or services:
One identical motor is used on Unit 2 at North Anna.
Identical motors are also used on North Anna Unit 1, Surry Unit 1, and Surry Unit 2.
G.
Corrective action which has been,.is being or will be taken, the indi-vidual responsible and the length of time to complete the action:
The motor will be returned to General Electric in San Jose, California for complete damage evaluation and repair.
All damaged parts will be replaced or repaired as deemed neces-sary by General Electric.
The lower end casting of the iden-tical motor installed in North Anna Unit 2 was field inspected, and no evidence of defectiveness was found.
Since the identi-cal motors in North Anna Unit 1 and Surry Units 1 and 2 have all operated in excess of thirty hours without incident, it is considered unlikely that a similar defect exists in their cast-ings.
The casting flaw appears to be an isolated case of gross shrinkage porosity rather than a generic imperfection.
Never-theless, visual inspections will be made on the identical motors used in North Anna Unit 1 and Surry 1 and 2 at the next outage.

Since no other actions will be required to correct the problem described in section D of this report, we consider this to be the final report under the provisions of 10CFR50.55(e)(3).
