TOLEDO EDIS0N COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL IN@fATION FOR LER NP-09-78-02 DATE OF EVENT: December 19, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: RE 744, Condensate Demineralizer Backwash Receiving Tank Monitor, was inoperable.
Conditions Prior to Occurrence: The unit was in Mode 3, with Power (MUT) = 0, and Load (Gross MWE) = 0.
Description of Occurrence:
On December 19, 1978 at 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br />, through conversation with the Instrument and Control Coordinator, it was discovered that RE 744, the Con-densate Demineralizer Backwash Receiving Tank Monitor, had been inoperable since August 11, 1978.
RE 744 was found to have had an inoperable low flow light during performance of Surveillance Test ST 5032.01, " Monthly Functional Test of the Radia-tion Monitoring System", on August 11, 1978.
The unit was not in compliance with Environmental Technical Specification Table 2.4.3 which _ requires the operability of the Condensate Dcmineralizer Backwash Receiv-ing Tank Radiation Monitor. No Action Statement was applicable.
Designation of Apparent Cause of Occurrence: The apparent cause of the occurrence is lack of proper flow through the RE due to low pressure in the sample line. A Facility Change Request has been written for resolution of this problem.
The reason for the delay in reporting the occurrence is due to improper interpreta-tion of Environmental Technical Specifications.
Analysis of Occurrence: There was no danger to the health and safety of the public or to unit personnel.
RE 609 and RE 600, Main Steam Lines 1 and 2 Radiation Monitors, respectively, which would also detect any steam generator tube leaks, have indicated no radioactivity.
In addition, grab samples of secondary plant side water (taken thrice weekly) have indicated no radioactivity.
Corrective Action: Work Request IC-78-2733 was written on August 14, 1978, to in-vestigate/ repair the cause of the low flow light on RE 744.
s On August 18, 1978, Facility Change Request 78-431 was written. This Facility Change Request requests the installation of a separate sample pump for RE 744, tak-ing suction from the bottom of the backwash tank and discharging at the top of the tank.
s TOLEDO EDISON COMPMIY DAVIS-BESSE NUCLEAR P0tlER STATION UNIT ONE PAGE 2 SUPPLEMEEiL INFORMATION FOR LER NP-09-78-02 5032.01 and ST 5032.02 to emphasize Major Modifications to Surveillance Tests ST reporting requirements of RE's in the Environmental Technical Specifications have been prepared. A memo dated January 4,1979, to various station personnel emphasizing these reporting requirements, has also been prepared.
The Environmental Technical Specifications are currently in the process of revision.
There have been no previously reported occurrences of the failure Failure Data:
of RE 744.
