APPENDIX A University of Hawaii at Manoa Hawaii Hall, 2500 Campus Road Honolulu, Hawaii 96822 License No. 53-00017-23 MOTICE OF VIOLATION Based upon the results of the special NRC inspection conducted on October 23, 1980, certain activities under your license appear to be in noncompliance with NRC requirements as identified below:

10 CFR 20.101(a) states that except as provided in 20.101(b), no licensee shall possess, use, or transfer licensed material in such a manner as to cause any individual in a restricted area to receive in any period of one calendar quarter from radioactive material and other sources
_of radiation a total occupational dose in excess of 1.25 rems (whole body).
Contrary to 10 CFR 20.101(a), a university employee received a whole body dose of 1.94 rem for the period May 1, 1980 - May 31, 1980.
This is an infraction.
