e APPENDIX A NOTICE OF VIOLATION Cincinnati Gas and Electric Company Docket No. 50-358 Based on the inspection conducted on January 3-6, and 8, 1979, it appears that certain of your activities were in noncompliance with NPC requirements as noted below. This item is an infraction.
10 CFR, Appendix B, Criterion V, states, in part that, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall te accomplished in accordance with these instructions, procedures, or drawings." Paragraph 17.1.5 of the FSAR states, in part, " Activities affecting the quality of the facility are accomp-lished in accordance with written instructions, procedures, or drawings.

Paragraph 6.6.5 of procedure EC. SAD.02, Revision 00, requires that the operator assigned to execute a switching order perform the actions stated in the switching order.
Switching Order 781450 executed on November 13, 1978, required valve IE12F053A to be closed.
Contrary to the above the operator who executed Switching Order 78145D on November 13, 1978, failed to ensure that valve IE12F053A was fully closed. A result, substandard water leaked into the Class B clean reactor vessel on December 13, 1978.

Paragraph 4.10 of procedure SU.ACP.03, Revision 3, assigns field quality assurance (QA&S) group the responsibility to assure post flush cleanliness is maintained on systems under GED construction jurisdiction.
Contrary to Criterion V the QA&S group has not developed procedures appropriate to assure boundary valves on a clean system are closed in order to prevent the introduction of substandard fluid (e.g.,
the occurrence on December 13, 1978 when substandard water leaked into the Class B clean reactor vessel).
