Appendix A (hh NOTICE OF VIOLATION Commonwealth Edison Docket No. 50-373 Company Docket No. 50-374 Based on the inspection conducted on January 10-12, 1979, it appears that certain or your activities were not conducted in conformance with NRC requirements, as noted below.

These items are inf ractions.

1.

10 CFR 50, Appendix B, Criterion X requires, in part, that "A program for inspection of activities affecting quality shall be established and executed by or for the organization performing the activity to verify conformance with the documented instructions, procedures.

. for accomplishing the activity." Section 10 of Commonwealth Edison Company (CECO) Topical Report, CE-1-A, endorsed by the FSAR Chapter 17, "QA Program for Nuclear Generating Stations,"

Revision 5, dated August 11, 1978, states, in part, " Inspection Program will be established to provide assurance that the quality control.

. inspections and tests defined in applicable equipment specifications are performed. QA inspection and testing will be conducted.

. at the site during construction.

to verify conformance to applicable drawings, instructions, and procedures as necessary to verify quality."

Contrary to the above, the inspection program for concrete expan-sion anchor bolts was not considered adequate in that (1) the extent to which anchor bolts can be installed out of plumb was not specified or inspected, (2) the minimum bolt embedment length was not defined or inspected, and (3) random NRC inspection sbowed 40E torque test failure on 5/8" bolts.

10 CFR 50, Appendix B, Criterion V requires, in part, that

" Activities af f ecting quality shall be prescribed by documented instructions, procedures.

." Section 5 of CECO Topical Report, CE-1-A, endorsed by the FSAR, "QA Program for Nuclear Generating Stations," Revision 5, dated August 11, 1978, states, in part, "The quality assurance actions carried out for.

. construction,

testing, will be described in documented instructions, proce-dures, drawings.

These documents will arsist personnel in assuring that important activities have been performed."

Contrary to the above, the rework and replacement program for rejected concrete expansion anchor bolts was not adequate in that (1) there was no procedural provision to ensure timely corrective action, and (2) there was no procedure for removing rejected bolts and installing new or replacement bolts.
