b Appendix A NOTICE OF VIOLATION Cleveland Electric Illuminating Docket No. 50-440..
Docket No. 50-441 Company Based on the results of an NRC inspection on November 29 -
December 1, 1978, it appears that certain of your activities were in noncompliance with NRC requirements as noted below.
Items 1 and 2 are infractions, and item 3 is a deficiency.

10 CFR 50, Appendix B, Criterion VII states, in part,
" Measures shall be established to assure that purchased materials, equipment and services, whether purchased directly or through contractors and sub-contractors, conform to the procurement documents."
Paragraph 17.1.7 of the Quality Assurance Program documented in the Perry Nuclear Power Plant (PNPP)
PSAR states, in part, " Appropriate measures will be included to evaluate procurement sources, monitor the activities of vendors and tantractors, and confirm that purchased material conforms to procurement documents.
Contrary to the abcve, measures established by CEI did not assure that stainless steel liner plates being installed in the Spent Fuel Storage Pool were inspected to Section 4.30 of the AWS D1.1-75 Code, as required by paragraph 2:07:2 of Specification SP-665.
The NRC inspector identified several Nelson Stud attachments which had been repaired by stick welding and had not been bend tested as required by the code.
Also, several welds attaching angle-iron stiffeners to the stainless steel weir wall liner plates were observed by the inspector to be less than the specified size.
CEI's failure to assure that purchased materials (embedments and structural steel) conform to require-ments was cited (against Criterion IX) once previously as an item.I noncompliance in IE Inspection Reports No. 50-440/78-02 and No. 50-441/78-01.

10 CFR 50, Appendix B, Criterion IX states, in part,
" Measures shall be established to assure that speelal-processes, including welding.
. nondestructive testing, are controlled and accomplished.... using qualified procedures in accordance with applicable codes, standards, specifications, criteria...
Paragraph 17.1.9 of the Quality Assurance Program in the PNPP PSAR states, in part, "Special processes to be used during the manufacture and installation shall be established and controlled in accordance with approved procedures. Welding,
.' nondestructive examination and other processes which require unusual care or close control shall be performed in accordance with appropriate written proc 6dures."
Contrary to the above, on the evening of November 28, 1978, the NRC inspectors observed that adequate ceasures had not been established to verify and document that the prehent temperature was above 70 F for containment liner plate welding, as required by the relevant specification, SP-17.
Furthermore, the inspectors determined that a program was not established to ascertain whether the welding machines met the requirements of paragraph 3.1.2 of the AWS Dl.1-75 Code or to document that the NNI inspectors verified that the requirements of paragraph 6.3.2 of AWS Dl.1-73 Code were met.
Specification SP-17 requires conformance to AWS D1.1-75.

10 CFR 50, Appendix B, Criterion XVII states, in part, that " Sufficient records shall be maintained to furnish evidence of activities affecting quality.
The records shall include
. the results of reviews, inspections, The records shall also include closely
- tests, related data such as qualifications of personnel."
Paragraph 17.1.17 of the Quality Assurance Program documented in the PNPP PSAR stceae, in part, " Approved procedures shall establish and control the QA records program of each participant responsible for quality."
Contrary to the above, the NRC inspector identified several discrepancies in the Pressure Test Reports related to the Units 1 and 2 Reactor Vessel Pedestal embedment leak chase systems.
