DOCKET 50-213 EXPOSURE REPORT IN ACCORDANCE WITH 10 CFR 20.405, 20.408 and 20.409 We are in receipt of your letter dated December 26, 1978 regarding Inspection Report 50-213/77-23 dated January 6, 1978. In accordance with your directions, we are herewith making a report in accordance with 10 CFR 20.405 of an in-dividual whose TLD dosimeter indicated that he might have received an exposure to the skin of the whole body of 23.50 Rem during the period 10/15/77 to 10/28/77.
If the TLD dosimeter reading were correct, this exposure would be in excess of the quarterly allowable level of 7.50 Ram.
This is however within the annual allowable 30 Rem level.
In our submittals to you dated Fdbruary 2, March 9 and November 27,1978, we explained that such a level of exposure to the individual was highly unlikely considering the contamination of the TLD device. We believe exposure to the individual's skin was 2.710 Rem as determined by the film badge dosimeter.
However, for the purposes of maintaining the individual's dosimetry record in the most conservative f ashion, the TLD dosimeter reading will be used.
We have notified the individual of this situation in a revision to his termina-tion letter. His NRC Form 5 will be revised to reflect the same information.
In our February 2, 1978 response, we described our intention to conduct a detailed investigation of this situation with the TLD vendor. An extensive QA audit was performed and recommendations for corrective actions were made to the vendor. Subsequently, we decided to use another TLD vendor effective July 1, 1978. The new vendor's dosimeter processing system was audited to 7903 09 06 t;5I
M,r. Boyce Grier January 22, 1979 minimize the potential for an occurrence such as in this incident. Within the plant, steps have been taken to reinforce the strict adherence to good health physics practices. These include increased control and ALARA review of radiation work permits, more vigilant health physics job coverage, and a greater use of portable continuously indicating radiation monitoring equip-ment.
