TRW f41ssion lianufacturing Company Docket rio. 99900287/78-02 fl0TICE OF DEVIATION Based on the results of an flRC inspection conducted on October 30 -
flovember 3,1978, it appeared that certain of your activities were not conducted in full compliance with flRC requirements as indicated below:
A.
The TRW liission fianufacturing Company corrective action response letter of liarch 27, 1978, states with respect to deviation A in IE Report flo. 78-01, " Failure to issue the committed Quality Control Inspection Instructions and Inspection Report was the result of our significantly underestimating the complexity of performing this task properly, for a wide range of components.
Our revised commitment date for issuance is June 30, 1978."
The commitment date for this item was further revised to August 1, 1978, by a TRW Mission fianufacturing Company letter dated May 22, 1978.
Contrary to the above, the committed Quality Control Inspection Instructions and Inspection Report had not beca issued as of this inspection.
This is a repeat deviation from corrective action conmitments.

B.
Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions...
and shal1 be accomplished in accordance with these instructions Paragraph 5.2 in Section 5.0 of the Product Assurance Meaual states in part with respect to Shop Travelers, "... They list the manufacturing or assembly operations, inspection or testing operations and manufacturing methods and identify specifications and procedures to be used by number and revision...." Paragraph 5.5 states in part, "At each production ope"ation the Shop Traveler will be signed as applicable by the Machinc Operator and/or the Quality Assurance representative...."
Paragraph 1.4.7 in Section 1.0 of the Product Assurance Manual states,
" Manufacturing shall be responsible for explicity following all Shop Travelers and other work defining documents."

. Contrary to the above, the inspector observed the following with respect to the Shop Traveler packet for an eight (8) inch valve body, Job No. 80015, QC No. 5999:
1.
A Repair Traveler, issued May 24, 1978, did not include the method or an operation for defect removal.
2.
Manufacturing did not explicitly follow the Repair Traveler, issued July 27, 1978, relative to operation sign-offs.

C.
Criterion VIII of Appendix B to 10 CFR 50 states in part, " Measures shall be established for the identification and control of materials, parts, and components... These measures shall assure that identification of the item is maintained,... throughout...
installation, and use of the item."
Paragraph 5.1.2.1 in Section 5.0 of the Product Assurance Manual states, " Items to be manufactured from barstock will carry heat code identification which shall be transferred from the barstock to the items and verified and documented in the Shop Traveler by Inspection."
Paragraph 5.1.4 states in part, "At Assembly, the Quality Control Process Serial Numbers and/or heat numbers for controlled items will be listed on the Valve Inspection and Test Report...."
Contrary to the above,,the inspector observed the following with respect to hinge pins, which are manufactured from barstock:
1.
Pins stored in the warehouse were identified only by a material type notation and not by heat code identification.
2.
Stainless and carbon steel pins were observed intermingled in a location designated as carbon steel.
3.
Identification of the hinge pin used in Valve Serial No. H7450, Job No. 62753 was not made in either the Shop Traveler or the Valve Inspection and Test Report.
D.
Criterion IX of Appendix B to 10 CFR 50 states in part, " Measures shall be established to assure that special processes, including
... nondestructive testing, are controlled and accomplished by quclified personnel... in accordance with applicable codes...."
Paragraphs NC/ND-5521 in Section III of the ASME Code state in part,
"... For nondestructive examination methods not covered by SNT-TC-1A documents, personnel shall be qualified by the Manufacturer er Installer to comparable levels of competency by subjection to comparable examinations on the particular method involved...."
_3 Contrary to the above, Quality Control Procedure No. 035 Revision A, Procedure for Visual Examination, permitted Level II magnetic particle or liquid penetrant personnel to perform visual examination without being subjected to a comparable examination in the visual method.
