ENCLOSURE 1 Nebraska Public Power District Cooper Nuclear Station Docket No. 50-298 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on January 29 -
February 1,1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
A.
Criterion V of Appendix B to 10 CFR 50 requires activities affect-ing quality be prescribed by documented instructions, procedures or drawings and be accomplished in accordance with these instructions, procedures or drawings.
This requirement is amplified in Section 2.5 of the licensee's approved Quality Assurance Manual.
Contrary to the above, licensee procedure 1.6, " Personnel and Equip-ment Safety," was not adhered to in that approximately two quarts of alcohol was found on January 30, 1979 on the refueling floor of the reactor building in a seven gallon open-topped plastic bucket instead of a metal safety can as required.
This is an infraction.
B.
Technical Specification 4.5.B.2 requires the remaining service water booster pumps (SWBP) to be demonstrated to be operable immediately upon rendering or determining that one SWBP is inoperable.
The definition of immediate contained in the Technical Specifications means as soon as practicable considering plant conditions and the importance of the required action.
Contrary to this specification, the 'B' SWBP was rendered inoperable for scheduled maintenance at approximately 0900 on November 19, 1978, however, the required surveillance on the remaining SWBP's was not performed until the oncoming (1600-2400) shift determined that the pump was inoperable.
This item is a deficiency.
