9 APPENDIX A NOTICE OF VIOLATION Worcester Polytechnic Institute Docket No. 50-134 Based on the results of an NRC inspection conducted on December 18-19, 1978, it appears that certain of your activities were not conducted in full compliance with the Code of Federal Regulations and your Facility License R-61 as indicated below.
Item A is an Infraction, Item B is a Deficiency.
A.
Technical Specification 9.4 states:
" Detailed written procedures shall be provided for all normal operations of the reactor, supporting facilities, maintenance operations, radiation protection and experi-ments and for emergency plans and operations.
These procedures shall be approved by the Radiation, Health and Safeguards Committee prior to implementation."
Contrary to the above, a detailed written procedure to accomplish regeneration of the pool water purification resins was not approved by the RHSC as of December 19, 1978.
This is a supporting facility maintenance operation, entails releases of caustics, acids, and potential radioactive water to the city sewers.
Regeneration was accomplished in January and September,1977, and January and August, 1978.
B.
Technical Specification 9.2 states, in part:
"A Radiation, Health and Safeguards Committee shall exist...
The Committee shall establish written procedures regarding review methods, quorums and sub-committees..."
The RHSC Procedural Rules, dated January 23, 1976, states, in part:
"The RHSC shall hold at least four meetings each year, at least one each in January, July, October, and April...
A quorum shall consist of four committee members..."
Contrary to the above, the quarterly meeting held on July 17, 1978 was held without quorum, in that only three committee members were present.
