License No. CPR-50 APPENDIX A NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on January 10-11 and 16, 1978, it appears that one of your activities was not conducted in full compliance with the conditions of your NRC Facility License No. DPR-50 as indicated below.

This item is an Infraction.

Technical Specification 6.8.1 states in part, " Written procedures and administrative policies shall be... implemented...that meet or exceed the requirements...of Section 5.1 and 5.3 of ANSI N18.7-1972 and Appendix A of USNRC Regulatory Guide.1.33...."

Surveillance Procedure 1301-9.9, Hydraulic Shock and Sway-Suppressors,

Revision 7, S'ep 6.5.1, states: " Insure all snubbers have been in-spected." Step 6.5.2 of the same procedure states: "Any inoperative snubbers (i.e., no fluid or out-of-position) must be reported to the Shift Supervisor immediately. The Shift Supervisor will then ir. form the cognizant personnel.

Note: If a snubber is found to be empty of fluid, it should not be refilled immediately.

If possible, it should be replaced with an equivalent spare unit and placed on the snubber test stand to determine its operability and to locate the cause of leakage."

Acministrative Procedure 1010, Technical Specification Surveillance Program, Revision 12, Section 4(b) states: "The personnel assigned go to their local file of controlled copies of surveillance procedures, remove the ' working copy' of the procedure assigned and xerox a copy of all data sheets associated with that procedure." Section 4(c) states: "The personnel assigned accomplish the procedure and fill in all applicable data and EaD sheet entries (if applicable)."

Centrary to the above, on May 7,1977, Snubber NSE 96A, which was found with no fluid, had been refilled without any testing to detennine operability and had no Exception and Deficiency (E&D) Sheet prepared.

Furthermore, on five different occasions, between March and Decemcer of 1977, data sheets were missing when performing snubb,er visual inspections in accordance with Surveillance Procedure 1301-9.9.
