License No. DPR-50 APPENDIX A NOTICE OF VIOLATION Based on the results of an NRC inspection conducted onsite February 26 -
March 1, 1978, and offsite on March 9, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC License No. DPR-50.
Items A and B are infractions.
A.
10 CFR 20.203(b), " Radiation Areas," requires that each Radiation Area shall be conspicuously posted with a sign or signs bearing the radiation symbol and the words, " Caution, Radiation Area," and 10 CFR 20,202(b)(2) defines a Radiation Area as any area accessible to personnel in which there exists radiation, originating in whole or in part within licensed material, at such levels that a major portion of the body could receive in any one hour a dose in excess of 5 millirem.
Contrary to the above requirement, on the morning of February 27,

1978, there was no conspicuously posted " Radiation Area" sign at either the Nuclear Sample Rocm or the personnel entrance to the reactor building - areas that were accessible to personnel, in which the radiation levels exceeded 5 millirem per hour.
B.
Technical Specification Section 6.11 requires adherence to pro-cedures for all operations involving personnel exposure.

Procedure HP 1612 requires individuals to monitor themselves for contamination on leaving a contaminated area, going to a controlled area.
Contrary to the above requirement, at 10:45 a.m., on February 27, 1978, an individual who was removing a 50 cubic foot radwaste container, leaving a contaminated area - the Radwaste Process Area elevation 305 feet, going to a controlled area failed to monitor himself for contamination.

Procedure HP 1620 requires a determination that solidified radwaste containers are free of contamination before they are moved frcm the Radwaste Process Area to a temporary storage area, and that Form 1620-1 be used for the survey.
Contrary to the above requirement, at 10:45 a.m., on February 27, 1978, a solidified radwaste container, No. 78-C-25, was moved from the Radwaste Process Area to a temporary storage area without any determination that the container was free of contamination before it was moved.
