- 1973, it _pcedred that certain of your activities v,ere not conducted in full compliance with NRC requirements as indicated below:
A.
The Babcock ar.d Wilcox - Control Components International's Director of Quality Assurance stated in his letter of February 13, 1973, to the NRC, Region IV, "..
CCI will reaudit this vendor.
This reaudit will be completed by March 10, 1973."
The inspector verified that contrary to the above co riitments to NRC, the vendor, a heat treating company, had not been reaudited.
B.
Criterion III of Appendix B to 10 CFR 50 states in part, "The design control measures shall provide for verifying or checking the adequacy of design.
Paragraph NCA-4134.3(b) of Section III to the ASME Code states in part, "... Design reviews and checking shall be perfor ed by or for the responsible design organi:'ation by individuals or groups other tnan those who perfor ed the original design."
Paragraph 11 of Procedure 1704-01 in Section 3 of BW-CC's ASME accepted Quality Assurance Manual states in cart, " Product Application Supervisor Reviews design and calculations for completeness and correctness.
Pethod of calculations verification shall be included in the design package.
Contrary to the above, the Product ?oplication Supervisor, in his review of the design and calculations for the lh" - 1500= Sel F Drag Valve Model No. 89G5-05-15SW-lam 42 for completeness and correctness did not identify that the calculations for the Disc Ce3 ring Area (page 2) had not been completed.
C.
Criterion III of Appendix B to 10 CR 50 states in part,
. The design control measures shall provide for verifying or checking the adequacy of design,
. by the use of alternate or simplified calculation methods...."
Paragraph NCA 4134.3(b) of Section III to the ASME Code states in cart Design Reports and the overall design shall be reviewed for design adequacy and comoliance with the Design Specification and this Section 790321o l T2_
Paragraph 11 of Procedure 1704-01 in Section 3 of E'A-CC's ASME acc u ted Quality Assurance Manual states in part, " Product Aaplication Supervisor
... Reviews design and c11culations for comcleteness and corractness.
Metnod of calculations verification shall be included in the design package.
Contrary to the above, method of calculations verification iur the bolting of a 13-15005 Self Drag Valve - Podel No. B9G5-05-15SW-14AA44 was not in the design package.
D.
Criterion VI of Appendix 3 to 10 CFR 50 states in part, "Peasures shall be established to control the issuance of documents, such as instructions, procedures, and drawings, including changes thereto, which prescribe all activities affecting quality.
These measures shall assure that documents, including changes, are reviewed for adequacy and approved for release by authorized personnel Paragraph NCA-4134.6 of Section III to the ASME Code states in part, "The Program shall include measures to control the issuance and disposition of documents, such as specifications, instructions, procedures, and drawings, including changes thereto.
These measures shall assure that the latest applicable documents, including changes, are reviewed for adequacy and approved for release by authorized personnel...."
Contrary to the above, there was no documentary evidence that The Calculational Procedure and Cesign Standard had been reviewed for adequacy and approved for release.
E.
Criterion IX of Appendix B to 10 CR 50 states in part, ""easures shall be established to assure that special processes, including welding,...
are controlled and accomplished by qualified personnel using qualified procedures...."
Paragraph WCA-4134.9(a) states in part, ".
. Measures shall be established to assure that processes, such as welding,... are controll-ed in accordance with the rules of this section and are acccmolished oy qualified personnel using qualified procedures."
BW-CC welding procedure specification, WPS No. HS-27-4, Revision A, for piasma-arc hardfacing requires the interpass temperature to be within the 700 to 9000F range.
Contrary to the above, it was observed that the above WPS was being used for hardfacing Part No. 120602091, on Work Orders 16900 and 16321, and Shop Order 21974-63-701, and the interpass temperature was not being monitored to verify ccmoliance witn the WPS requirement.
