  • ENCLOSURE SEQUOYAll NUCLEAR PLANT UNITS 1 AND 2 INSPECTION REPORT 327/78-41 RESPONSE TO INFRACTIONS 78-41-03 AND 78-41-04 Infraction 78-41-03 Criterion IX of Appendix B to 10 CFR 50, as implemented by the FSAR, paragraph 17.1A.9, requires in part, " Measures shall be established to assure that special processes, including. . . nondestructive testing, are controlled and accomplished. .

The ASME Boiler and Pressure Vessel Code, 1974 edition plus addenda through the summer of 1975, Appendix I, paragraph I-4230 requires daily UT instrument calibrations for amplitude linearity and amplitude control linearity for UT of ferritic pressure vessels 2 1/2-inches and greater in thickness.

Contrary to the above requirements, the following was observed: on December 13, 1978, during inspection of the UT procedure applicable to ferritic pressure vessels 2 1/4-inches and greater in thickness (UT-2, Rev. 11), the inspector noted that the requirement for daily linearity calibrations was not delineated in the procedure.

Response to Infraction Linearity calibrations on ultrasonic equipment are now being performed on a daily basis. Further problems will be prevented by changing Lambert, MacGill, and Thomas (LMT) procedure UT-2, Revision II, to require daily linearity calibrations. The new procedure will be issued by February 5, 1979.

Infraction 78-41-04 Criterion V of Appendix B to 10 CFR 50, as implemented by the FSAR, paragraph 17.1A.9, requires in part, " Activities af fecting quality shall be prescribed by documented instructions, procedures. . . and shall be accomplished in accordance with these instructions, procedures

. . . ." Ultrasonic testers shall. . .have their proper internal alignment and calibration verified within 50 days of use."

Contrary to the above requirements, the following was observed: on December 11, 1978, the inspector observed UT tester serial no. 130-063-D being used for testing pressurizer nozzle to vessel welds RCW-15, 18 and 20.

On December ic 1078, during review of calibration certification for this UT tester. the inspector noted that the most recent calibration was performed on September 6, 1978.

  • 4 6 Response to In f r.ict ion The UT instrument found out of calibration was immediately removed from use on December 14, 1978, and sent to be recall' rated. The instrument was checked in the as-found condition and was found to meet all manu-facturers' specifications. It was then recertified and placed back in service. Future occurrences will be prevented by assuring that all personnel involved are aware of the 90-day internal alignment and calibration requirement.
