BOX 361 PLATTEVILLE, COLORADO 80651 REPORT NO. 50-26 7/ 79-04/ 03-L-0 Final IDENTIFICATION OF OCCURRENCE:

On February 1,1979, two violations of Technical Specification LCO NR 1.3 were discovered to have occurred during the previous year. Failure to report these events is a reportable occurrence per Fort St. Vrain Technical Specifi-ation AC 7.5.2(b)2.

EVENT DESCRIPTION :

The total quantity of cooling tower blowdown, as determined by the difference la blowdown integrator readings taken at 0000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> one day to 0000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> the next day, exceeded the limit of LCO NR 1.3 which is 3. 75 million gallons per day (MGD). On June 14, 1978, the blowdown was 4.449 MGD. On December 12, 19 78, the blowdown was 3.84 MGD.

These violations were discovered during an audit of surveillance records.

The surveillance form did not clearly call out what deficiencies were to be reported and chemistry personnel did not correctly complete the form.

CAUS E DES CRIPTION ;

The cause of the excessive blowdown rates are undetermined, but these appear to be unrelated incidents as the chemistry reports for both days fail to in-dicate any need for excessive discharges and all chemical concentrations were within acceptable linits.

The cause for not reporting the incidents when they occurred was incurrect completion of the surveillance form by chemistry personnel. This could, in part, be attributed to the surveillance form not being clear on what should be reported as a deficiency and the actual LCO limit not appearing on one of the data sheets.

i REPORTABLE OCCURRENCE 79-04 ISSUE O Page 2 of 2 CORRECTIVE ACTION:

The excessive blowdown rates were corrected the following day. The surveil-lance form was revised to show the LCO limits clearly and the chemistry per-sonnel were informed of the correct method for marking deficiencies on the surveillance forms.

No further corrective action is anticipated or required.
