O Ametek Corporation Calmec Division Docket No. 99900112/78-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on October 2-6, 1978, it appears that certain of your activities were not conducted in full accordance with NRC requirements as indicated below:

A.

Criterion II of Appendix B to 10 CFR 50 requires that the program provide for indoctrination and training of personnel performing activi-ties affecting quality as necessary to assure that suitable proficiency is achieved and maintained.

The QA Manual CM-615, paragraph 1.4.1, states in part, " Personnel per-forming activities affecting Quality shall be trained and indoctrinated as necessary to assure suitable proficiency is achieved and maintained Paragraph 1.4.3 states in part, " Personnel qualification records, as well as records describing training and indoctrination sessions, shall be maintained by quality assurance...."

Contrary to the above, Quality Assurance did not maintain records of auditor indoctrination and training.

B.

Criterion XM of Appendix B to 10 CFR 50 requires that measuring and testing de, ices used in activities affecting quality are properly con-trolled and calibrated at specified periods to maintain accuracy within necessary limits.

The QA Manual CM-615, paragraph 12.1.4.4 states, "The Inspection Super-visor shall re-evaluate periodically to assure that the established calibration cycle is sufficient to maintain accuracy and proper function-ing of the equipment." Paragraph 12.1.9.2 states, "The calibration shall be performed in accordance with Ametek/Calmec Quality Control Calibration Procedure CM-1005." Procedure CM-1005 requires that temperature indicators be calibrated every 6 months.

The QA Manual CM-615, paragraph 12.1.9.6 states in part, "The calibra-tion sticker shall indicate the date of calibration, the due date of next calibration...."

Paragraph 12.2.1.2 states in part, with regard to calibration record cards, "The individual cards, shall indicate the complete history of the itea including... date of last calibration,... due date of next calibration...."

790302 C # 8 Contraty to the above, the following conditions were observed:

1.

Calibration frequencies for equipment listed in Procedure CM-1005 have not been re-evaluated since June 15, 1972.

2.

The temperature indicator No. 2020, on the weld preheat oven, had exceeded its six (6) months calibration frequency by two (2) months in that the calibration due date was August 8, 1978.

Note: During the inspection on October 4, 1978, the above indicator was replaced with a temperature indicator which was in calibration.

3.

The tool calibration card dates for the master pressure gages SN-3001 and 3002 did not match their sticker's calibration dates.

The cards required a semi-annual calibration cycle and showed a calibration date of July 12, 1977, with a due date of January 12, 1978. The stickers on these gage; showed that the gages were calibrated on August 8,1978, and had a recalibration due date of August 8,1979, indicating an annual calibration cycle instead of a semi-annual calibration cycle.
