The inspection involved a total of 46 inspector- hours onsite by two NRC inspectors. Results: Of the four areas inspected, one item of noncompliance was identified regarding failure to follow " instructions" relative to personnel airlocks repair velding. (An infraction) 7903020210

Other personnel of the licensee and his contractors were contacted during the inspection. Licensee Action on Previous Inspection Findings (open) Non c on,p l ianc e : Lack of control f welding processes includir, voltage. In respc.nse to questioning, the licensee informed the inspector that the procedure for monitoring of voltage and current of the welding machine during production welding was in draft status. However, it was indicated that the selective monitoring vas to be done on the basis of selecting a percentage of the cualified weldors each month for monitoring. After discussion, it was understood that this method fell short of the desired goal in that.this method did , _2_
. t not assure monitoring of all welding machines and in fact only guaranteed monitoring of one machine. Further review of this matter will be performed during future inspections. .- - Reportable Deficiencies Review was conducted of the 10 CFR 50, Section 50.55(e), deficiency report regarding both Unit 1 and 2 personnel airlocks weld crackinr. Bechtel Noncomformance Report No. 1185, Revision 3, providing the cracking repair disposition (including the W. J. Woolley & Co. Uc i t Repair Procedure No. S01011), was reviewed by the inspector. The completed welds of both airlocks were visually examined and it was observed that the completed welds for the stiffeners of both units, that required splice plates, were different, with Unit I welding having fillet welds that wrapped around the ends of the splice plates while Unit 2 had no welding at the end. During discussions, it was learned that the welding procedure instructions provided by NCR 1185 specifically called for fillet welding on both sides but not on the end. The inspector informed the licensee that this failure to follow instructions was considered to be contrary to 10 CFR Part 50, Appendix B, Criteria V, which requires in part that " Activities affecting quality shall be. . accomplished in accordance with. . instructions, . . procedures, or drawings." Prior to leaving the site, the inspector additionally learned that the licensee representative had placed these welds under nonconformance control by issuance of NCR No. M-01-9-8-096 dated November o, 1978, and that Rechtel had also issued an NCR No. 1636 regarding fillet weld size Functional or Program Areas Inspected Safety related piping fabrication activities were reviewed for work and quality assurance activities relative to two piping systems. 1. Make-up and Purification system. Welds reviewed included: -M-603 Sheet 12 (FW-12) -M-603 Sheet 12 (FN-3) -FS-K-M-1-HCC-7 (FW-8, 9, 10 and ID .- . , -3-
. . ' Weld records inspected indicated that: a. The NDE required by specification were transcribed to the , weld control record sheet. _ b. Non destructive examinations were performed as required kr
the weld control sheet. c. Non destructive examination personnel performing inspections of these welds were qualified. The records of qualification provided evidence of proner qualification. 2. Component Cooling Systems. Work activities concerning two welds were reviewed (FSK 'i-2 HRC- 128-1-12 and FSK-M-1 HRC-155-1-25) . Weld records inspected indicated that: a. For weld FSE-M-2HRC-128-1-12 the correct filler metal specified (E-705-2) was withdrawn from the filler metal storage areas and used by the welder. b. The correct non destructive test inspections were performed. The welder was currently qualified to perform welding to c. the welding procedure specified. d. For weld FSK-M-1 HRC-155-1-25 the pipe joint had been fit up but not welded. The work and inspections specified, such as the cleanliness inspection, had been performed as recuired. Exit Interview Ihe inspectors met with the individuals so denoted, under Persons Contacted, on November 9. 1978, at the conclusion of the inspection.
