t MAY 131980 APPENDIX A NOTICE OF VIOLATION Union Boiler Company License No. 47-16182-01 Huntington Test Division Based on the NRC inspection April 10, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by 10 CFR 34.44, whenever a radiographer's assistant uses radiographic exposure devices, he shall be under the personal supervision of a radiographer. The personal supervision shall include the ability of the radiographer to give immediate assistance if required and the radiogra-pher's watching the assistant's performance of the operations referred to in this section.
Contra ry to the above, on April 10, 1980, the day of the inspection, a radiographer's assistant was observed making a radiographic exposure on an elevated length of pipe, at the Monsanto Plant in Nitro, WV, while not under the personal supervision of the radiographer. During this operation the radiographer was some distance away in the Huntington Testing Division truck.
This is an infraction.
B.
As required by Condition 15 of your license, licensed material shall be possessed and used in accordance with statements, representations and procedures contained in your license application dated June 12, 1974 and in your letter dated September 23, 1974. Your letter dated September 23, 1974 requires that when licensed material is shipped to a field site, your driver will be furnished a copy of your Emergency Procedures.
Contrary to the above, it was determined that your radiographic crew per-forming work with licensed material at the Monsanto Plant in Nitro, WV on April 10, 1980, the day of the inspection, had not been furnished Emergency Procedures.
This is an infraction.
C.
A required by Condition 15 of your license, licensed material shall be possessed and used in accordance with statements, representations and procedures ccatained in your license application dated June 12, 1974 and in your letter dated September 23, 1974. Your letter dated September 23, 1974 requires that your quarterly Internal Audit of Radiographic Operations be recorded.

This is a deficiency.
D.
As required by 10 CFR 20.401(b) each licensee shall maintain records showing the results of surveys required by 10 CFR 20.205(c).
10 CFR 20.205(c) requires that each licensee, upon receipt of a package containing quantities of radioactive material in excess of the Type A quantities specified in paragraph (b) of this section, shall monitor the radiation levels external to the package.
Contrary to the above, it was determined that no records were available of surveys performed on incoming packages containing licensed material received since January 10, 1979. Specifically, the inspector observed on April 10, 1980, the day of the inspection, that there were four Ir-192 sources on hand which would have been received during that period.
This is a deficiency.
