8 0 05 071/2 g License No. 35-15727-01 NOTICE OF VIOLATION Based on the results of the NRC investigation conducted on November 7, 20, 21 and December 3, 4, 1979, certain activities appear to be in noncompliance with NRC regulations and the conditions of your NRC License No. 35-15727-01 as indicated below:

1.

License Condition 19 requires licensed material be used in accordance with statements, representations and procedures contained in license applications dated July 24, 1978, and October 21, 1978, and the attached document entitled " Star Jet Services, Inc. Operators Radioactivity Manual."

A.

The "Erar Jet Services, Inc. Operators Radioactivity Manual,"

Appendix C, IX, 3(b), states that no licensee shall dispose of radioactive materials by burial without a permit obtained from the Federal or State authorities.

Contrary to the above, rags contaminated with radioactive material were disposed of by burial at the Stockton-Thiele No. I well site on September 17, 1979.

This is an infraction.

B.

The " Star Jet Services, Inc. Operators Radioactivity Manual,"

Appendix C, VII, 2, states the Emergency Procedures to be followed for radiation hazard spills.

Contrary to the above, the survey after decontamination and history of spill portions of the Emergency Procedures were not followed for a radiation hazard spill which occurred at the Blackwell office during the Fall, 1979.

This is an infraction.

C.

The " Star Jet Services, Inc. Operators Radioactivity Manual,"

Appendix A, III, B states that handling tongs shall be used at all times when handling radioactive material.

Contrary to the above, a handling tong was not always used when a nominal 2.7 curie, Americium-Beryllium, sealed source was placed on the well logging tool during the period August 23, 1977, to November /, 1979.

This is an infraction.

The " Star Jet Services, Inc. Operators Radioactivity Manual,"

Appendix A, III, states that all personnel directly related to activity involving radioactive materials will wear a film badge.

Contrary to the above, personnel involved with radioactive material did not always adhere to film badge wearing instructions during the period August 23, 1977, to November 7, 1979.

Specifically, an individual who began work in April, 1979, was not issued a film badge unt'l August, 1979; tadividuals were not required to wear their film badges during operations involving licensed material; and film badges assigned to specific

's individuals were worn by other individuals.

E.

The " Star Jet Ser' ices, Inc. Operators Radioactivity Manual,"

Appendix B, III arid Appendix C, VIII, require radiation surveys of storage areas, vehicles, well sites, and personnel.

Contrary to the above, the storage area, vehicle, preoperational well site, post operational well site and personnel surveys were not performed on August 23, 1979, at the Corley Gio.her Oil Corporation job (job ticket number 800-2).

This is an infraction.

F.

The " Star Jet Services, Inc. Operators Radioactivity Manual,"

Appendix A, VI, requires a utilization log showing distribution of radioactive material, and receipt and disposal records be maintained.

Contrary to the above, a utilization log showing distribution of radioactive material, receipt records for shipments of radioactive material and, disposal records were not maintained during the period August 23, 1977, to December 4, 1979.

This is a deficiency.

2.

License Condition 12 requires licensed material be used in the physical presence of H. W. Patton, John Patton, Michael Deal, J. H. Davis, Ted Chappell, or Micheal Prange.

Contrary to the above, licensed material was used without the physical presence of any of the listed individuals on September 18, 1979, at the Christensen E#5 well site in Nowata County Oklahoma.

This is an infraction.

Contrary to 10 CFR 19.12, all individuals working in or frequenting a restricted area were not instructed in the health protection problems associated with exposure to radiation or in precautions and devices to minimize exposure during the period August 23, 1977, to November 7, 1979.

This is a infraction.
