Square D Company Assembly Se-vices Division Docket No. 99900367/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on June 25-28, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shs'l be prescribed by documented instructions, procedures, or drawings, of a type ap;ropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:
A.
Quality Control Procedure No. 120, dated November 27, 1978, which states in part, "If the material is accepted the inspector shall sign the move ticket and remove the white copy. He shall also sign and date the ' Delivery - Final Receipt.' copy of the purchase order."
Contrary.tg the above, the inspector had not signed and dated the Delivery - Final Receipt copies of the following purchase orders:
Material Received 12-24420 June 20, 1979 12-24901 June 20, 1979 12-24327 June 21, 1979 12-25078 June 21, 1979 Note: The above documents did exhibit an Inspection Stamp.

Paragraph I.D.I.b of Standard Practice: Bulletin (SPB) No. 100.001, dated October 17, 1972 (page 4), and June 13,1972 (page 5), which states, "The new, additional or revised paragraph or sub paragraph will be indicated by a straight vertical line to the right of the paragraph being changed."

2 Contrary to the above, straight vertical lines had not been used to indicate changes to SPB Nos. 100.013 revised November 9, 1978, and 512.404 revised August 19, 1976.
C.
Standard Practice Bulletin No. 512.404, dated August 19, 1976, states,
" Rework disposition must be completed within 10 working days."
Contrary to the above, rework dispositions had not been completed within ten (10) working days for parts 30307-110-01 and 30317-178-04 identified on Reject Tags dated March 15, 1979, and May 16, 1979, respectively.
D.
Corporation Quality Assurance Manual, dated May 16, 1978, which states in part, " Customer inspection hold points shall be documented on shop work orders or routing sheets as appropriate and written procedures shall provide for the prevention of further work until the inspection is complete and the material, components, or modules are identified as outlined."
A customer letter to Square D Coiapany, dated February 9, 1977, establishes hold points at:
" Final Electrical Testing and Final Inspection prior to Packaging"; with the requirement that the customer be notified two (2) weeks in advance of the activities.
Contrary't6'the above, customer hold po'ints had not been documented on shop work orders, routing sheets or other contractor documents for Motor Control Center sections electrically tested on October 12, 1977.
