This refers to the September 12, 1979 discussion between you and Mr. T.

Stetka of this office about the failure to incorporate instrument root valve lineup in the facility valve lineup control procedures as required by item 8 of IE Bulletin 79-08 prior to starting up the reactor plant.

3 With regard to the matters discussed, we understand that you have undertaken or will undertake the following actions:

1.

Suspend power escalation until the instrument root valve lineup has been accomplished and independently verified to be proper.

2.

Complete Plant Operations Review Committee review of the revised instrument valve lineup requirements on Sep'cember 13, 1979.

3.

Complete procedural revisions needed to incorporate instrument root valve lineups and issue the revised procedures by September 19, 1979.

4.

Conduct a specific review of the management control system which allowed this situation to occur, a1d make a written report of the results of that review to this office by September 27, 1979, with that report to place special emphasis upon the specific actions to be taken to prevent recurrence.
