Response to Inspection No. 50-309/79-03 In reply to reference (b), the Maine Yankee Atomic Power Company hereby submits the following information.

Item A., Apparent Item of Noncompliance:

Technical Specification 3.17.A.3 states, in part "The rate of release of radioactive materials in gaseous waste from the plant

...shall be controlled such that the maximum release rate averaged over any one hour period shall not exceed.. 31, 000 cubic meters /second."

Contrary to the above, samples of gaseous radioactivity being released from the Primary Vent Stack between 1:00 p.m. and 2:45 p.m.

on March 15, 1979, indicate a release rate of 78,604 cubic meters!

Technical Specification 5.8.1 states, in part, " Written procedures shall be established, implemented, and maintained covering...

the applicable procedures recommended in Appendix A of Regulatory Guide 7 9 0 8 02 010C'

Regulatory Guide 1.33 lists, in part, "3.0 Auxiliary or Reactor Building Heating and Ventilation", Plant Procedure 1-12-3,

" Control Area Ventilation", requires in paragraph 2.6 that, " Spray Building Ventilation exhaust be directed through the filters when-ever airborne activity exceeds 5 E-10 mC1/cc..."

Contrary to the above, based on air samples taken in the Spray Building between 1:00 p.m. on March 15, 1979, and 2:40 a.m. on March 16, 1979, airborne activity was as high as 2.8 E-5 mC1/cc and a review of the Control Room Log indicated that the ventilation exhaust filters were bypassed until 2:40 a.m. on March 16, 1979.

Item B., Reply:

To prevent the reoccurrence of this type of incident, two (2) plant procedures have been revised to ensure the Spray Building venti 11ation system is aligned through the HEPA and charcoal filters whenever conditions indicate that high airborne activity levels may exist.

Specifically, Emergency Procedure #2-25, Excessive Radiation Levels, was revised to require the operators to verify the status of the Spray Building ventillation system and ensure that flow is through the filters.

Additionally, Operating procedure #1-13-1, Residual Heat Removal Startup and Operation, has been revised to include an Initial Condition, prior to placing the RHR system into operation, of establishing the Spray Building venti 11ation system lineup through the filters. This lineup will remain in effect any time the RHR system is in operation through the Spray Build-ing.

These revisions have been reviewed, approved and distributed into the appropriate plant manuals and files.
