h ENCLOSURE 1 Arkansas Power and Light Company Arkansas Nuclear One, Unit No. 1 Docket No. 50-313 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted November 20 -

December 17, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your Operating License No. DPR-51 as indicated below:

Technical Specification 6.8.1 requires in part that, " Written procedures shall be established, implemented and maintained covering the activities referenced below:

"c.

Surveillance and test activities of safety related equipment."

Technical Specification 6.8.3 states in part that, " Temporary changes to procedures of 6.8.1 above may be made provided:

"b.

The change is approved by two members of the plant staff. at least one of whom holds a Senior Reactor Operator's License on the unit affected."

Contrary to the above, temporary changes were made to the accuracy requirements and to the desired output of Appendix A of Procedure 1304.30, Hydrogen Purge Analyzer and Flow Indication Instrument Eurveiliance, Rev. 4, without the required approvals.

The changes were made on data sheets for instruments SQT-7441, SQT-7442, 3QT-7451 and SQT-7452 on or about March 17, 1978.

This is an infraction.

ENCLOSURE 2 Arkansas Power and Light Company Arkansas Nuclear One, Unit 2 Docket No. 50-368 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted November 20 -

December 17, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your Operating License No. NPF-6 as indicated below:

Technical Specification 6.10.1 requires in part that, "The following records shall be retained for at least five years:

"d.

Records of surveillance activities, inspections and calibra-tions required by the Technical Specifications."

Contrary to the above, no records were retained which would sub-stantiate that changes to the steam generator low pressure trip and pressurizer pressure low trip settings were made in accordance with procedures and with the requirements contained in Combustion Engineering letter A-CE-7280.

This is a deficiendy.

Technical Specification 6.8.1 states in part that, " Written procedures shall be established, implemented and maintained for the activities referenced below:

Surveillance and test activities of safety rel,ated equipment."

Plant Procedure 1004.128, Revision 0, Operational Test Control, states in part wi6. regard to surveillance test Scheduling, "To ensure that required tests are performed within the specified interval, each Plant Supervisor designated on the Master Test Control List shall be

.t responsible for establishing and maintaining a departmental test control chart.

The test control chart shall list all required tests under the cognizance of that group, the test interval, scheduled date and date actually performed."

Contrary to the above, the inspector found that the test control charts were not being maintained current.

This is an infraction.

Technical Specification, Table 4.3-4, Item 3, specifies that the Triaxial-Response-Spectrum recorder 2XR-8350 is to receive a

" Channel Functional Test" on a semiannual "SA" frequency."

Contrary to the above, the interval of functional testing for 2XR-8350 had exceeded six months between the period of April 5,1978 to December 8, 1978.

This is an infraction.
