APPENDIX A NOTIJE OF VIOLATION United Nuclear Corporation Docket Nos.70-820 03013203 Based on the results of an NRC inspection conducted on February 28, 1979 through March 2,1979, it appears that certain of your activities were not conducted in full compliance with the conditions of your license as indicated below. All items are infractions.
A.
License Condition No. 9 of Special Nuclear Material License No. SNM-777 incorporates Section 200 of the licensee's application.
Section 205.1 states " United Nuclear Corporation policy requires that supervision at all levels assure themselves:
... that all pertinent regulations, controls and procedures relative to... radiological safety are fol-lowed by supervision and all operating perso nel."

Safety Work Permit No. I-N, dated August 7, 1976, developed pursuant to the above, states " Safety Work Permits are required wheneve,r any piece of equipment or associated piping within the process area is scheduled for maintenance, electrical or mechanical work and there is any possibility of releasing vapors, chemicals or airborne contamination as the result of the work being performed."
It also states that " Quality Assurance shall act as an overcheck to insure all aspects of the review have been considered.
Either the Manager, QA, or the QA Engineer may perform this overcheck and then issue the Safety Work Permit."
Contrary to the above, Special Work Permit No. 320 was issued on January 9,1979, and work was performed without the required overcheck by the Manager, QA or the OA Engineer.

Section 208.3 of the licensee's application states "The training and personnel safety program is continued with on-the-job training supplemented by regularly scheduled meetings..."
The Health Physics Manual, Revision 1, Section 2.3, developed pur-suant to the above, states " Annual retraining sessions... will be conducted by NIS."
Contrary to the above, approximately ten licensee employees did not receive annual retraining during 1978.

License Condition No. 9 of Special Nuclear Material License No.
SNM-777 incorporates Section 400 of the licensee's application.
Section 404.2.1 requires a minimum face velocity of 100 feet per minute for Fume Hoods and Section 404.2.3 requires a minimum face velocity of 150 feet per minute for General Purpose Hoods.
Contrary to the above, the specified minimum face velocity was not being maintained in two hoods as indicated below on March 1,1979:
Measured Face Velocity Hood in feet per minute Fume Hood in Chemistry Laboratory 60 General Purpose Hood (1-L-38) 130 C.
License Condition No. 9 of Special Nuclear Material License No. SNM-777 incorporates Section 400 of the licensee's application.
Secticn 401.1.1 of the application requires the licensee to take certain corrective action if beta contamination exceeds certain specified levels in re-stricted areas.
Contrary to the above, the licensee does not make surface contamination surveys to determine the beta contamination levels in restricted areas according to a licensee representative on March 2,1979.
