This license authorized the use of iridium-192 sources for industrial radiography at temporary job sites of the licensee anywhere in the United States where the NRC maintains jurisdiction.

A Certificate of Disposition of Materials (Form NRC-314) dated 8/29/79 and signed by Mr. R.R. Hertz' r stated that all materials were transferred to the State of California license 0519-90.

The NRC license expired on October 31, 1979.

Based on an application dated September 19, 1974 and signed by Mr. Earl Raymond, Vice President, Base Maintenance for United Airlines, License 04-03519-03 was issued on October 24, 1969 United Air Lines Maintenance Base; San Francisco International Airport; San Francisco, California 94128.

This license autrarized the use of iridum-192 for industrial radiography at temporary job sites of the licensee anywhere in the United States where the NRC maintains jurisdiction.

The license was amended six times.

Amendment 1 allowed the use of source changers.

Amendment 2 implemented a new set of operating and emergency procedures. Amendment 3 entirely amended the license and stated that the expiration date was October 31, 1979, The fourth amendment deleted the requirement that radiographers and assistants have an annual physical examination and semi-annual blood count.

Amendment 5 ' revised the operating and emergency procedures to state the name of a new Radiation Safety Officer and a new equipment manufacturers representative. Amendment 6 added a new alternate radiation safety officer.

The license was inspected on six occasions:

March 18, 1971 March 20, 1973 June 5, 1974 June 19, 1975 December 14, 1976 November 17-22, 1977 The inspection conducted in November of 19'/7 resulted in one item of noncompliance:

an exposure report of a terminated individual had not been submitted to the NRC. The report was submitted to NRC headquarters on November 21, 1977.

No other items of noncompliance were found as a result of other inspections.

An inspection of this license was attempted on November 22, 1978 but all activities had been conducted under the State of California License 0519-90. The license was deemed uninspectable.

The licensee filed a Form NRC-314 on August 29, 1979 stating that all future activities would be conducted under the state license.

On September 26, 1979, Mr. Lloyd Norton, Assistant RSO, called the Region V Office to discuss the expiration of the NRC license.

He stated that he had already submitted the Form NRC-314 tc NRC Headquarters.

Mr. Norton was advised to file 20.407 and 20.408 taformation-with Headquarters.

10 CFR 150.20 was also discussed.

The license expired on October 31, 1979, 20.407 data for the calendar year 1979 was subnitted on February 1, 1980.
