O'Reilly, Director May 3, 1979 IE INSPECTION REPORT 50-370/79-08 APPENDIX A NOTICE OF VIOLATION Duke Power Company License No. CPPR-84 McGuire Nuclear Station Based on the NRC inspection March 13-15, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

A. As required by Criterion V of Appendix B to 10CFR 50, as implemented by DPC Topical Report " Duke 1-A" Section 17, Paragraph 17.1.5 specifies in part that Activities affecting quality shall be accomplished in accordance with the requirements imposed by . . . instructions, procedures and drawings. QA pro-cedure M-4, paragraph 4.2 states in part that the welding inspector shall inspect the (weld) fitup including weld preparation and fitup dimensions; also paragraph 5.4 of QA Procedure F-9 requires the craftsman te record appropriate information on form M-4A and, that no additional work to be per-formed until the (weld) inspector has initiated the hold point on the process control record.

Contrary to the above, on March 15, 1979, the fitup of weld NC2FW15-2 had been changed from a consumable insert to open butt. However, the QC inspector who inspected the fitup failed to make the necessary changes on the weld record after accepting the new fitup.

This is a deficiency.

B. As required by Criterion XVII of Appendix B in 10CFR 50, as implemented by DPC Topical Report " Duke 1-A" Section 17, Paragraph 17.1.17 specifies that Quality Assurance records are maintained on site at a location where they can be reviewed to establish that the required quality had been assured.

Contrary to the above, on March 15, 1979, the licensee could not locate veld tickets and/or post veld heat treat charts for equipment hatch welds No. II-EH-1, -4, -5, -6, -9 and -10.

This is a deficiency.

Response

A. On weld MC2FW15-2 the craftsman first decided to make the weld using an EB ring. The EB ring heat number was placed on the M-4A by the welder.

After it was noticed that a proper fit-up could not be obtained, the EB ring was cut out and the joint was fit-up open-butt. This was done without the welding information for the EB ring being crossed out and a note explain-ing what happened. When the inspector came to inspect the fit-up, he signed of f the EB ring heat number and the fit-up as being acceptable. Nonconform-ing Item Report No. 8739 has been originated to address the above problem.

The welding inspector and welder have both been instructed to follow proper sequence of filling out documentation.

We feel that this is an isolated case which has been corrected and should not occur again subsequent to the reinstruction given the inspector and craftsman.

B. The welds with missing weld records were accepted based on the following:

a. Stencil numbers of welders who made welds are present on the welds.

These welders are qualified to make these welds.

b. Records of welding material issued to these welders are available.

These 10ts of material are acceptable for the welds,

c. The welding inspector involved signed a statement indicating that he performed all required inspections on the welds.
d. The NDE inspector involved signed a statement indicating that he per-formed the required weld root NDE. Surface NDE will be performed again.

Required radiographs are on file and acceptable.

The welds with missing post weld heat treatment charts were accepted based on the following:

a. The welding inspector signed a statement indicating that he reviewed the charts for all requirements. In addition, a welding technician reviewed four of the six charts for acceptance.

b. The hardness of the welds was measured and found to indicate that the welds were heat treated.
c. Records are available indicating that temporary welds were made and removed for the attachment of thermocouples and heating equipment.

Quality Control and Quality Assurance personnel will be instructed to maintain proper control of process control paperwork.
