Dear Mr. O'Reilly:
Due to an inadvertent typographical error, a portion of our previous submittal of this event report, May 16, 1979, was omitted.
The following is a resubmittal of this report in its entirety.
A shipment of radioactive waste left the Brunswich site by sole use vehicle on May 3,1979, with radiation levels up to 20 mr/hr at two meters (6 feet). This condition violates 49 CFR 173.393 (j)
(3), which states that the six-foot reading will not exceed 10 mr/hr.
This report is submitted in accordance with Environmental Technical Specification 5.4.2b.
Event
Description:
A shipment of 15 spent resin drums was loaded into a Chem-Nuclear cask ((!15-160) behind the Radwaste Building on the afternoon of May 3, 1979. The loading was completed at approximately 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br />.
A survey was made on the outer surface of the cask prior to movement.
Radiation levels on contact with the cask were less than 200mr/hr.
The truck carrying the cask was then moved outside the plant restricted areas boundary to await a final radiatica survey and completion of shipment loading.
Normally, the shipment documentation (resin batch number, drum activity, dose rate measurements, etc.) is initiated at the Radwaste loading dock. The documentation is returned to the Health Physics office for review by a second Health Physics technician following the loading; the truck is then placarded and moved outside the restricted areas boundary for a final radiation survey. A technician is dispatched from the Health Physics office to perform this survey.
Upon completion of the survey, the documentation is reviewed and completed, usually in the Health Physics office, immediately prior to the shipment leaving the plant site.

Mr. James P. O'Reilly June 1, 1979 In this incident, loading and survey proceeded normally until the time of the initial documentation review. The documentatien was reviewed and signed in the Health Physics office, then carried out with the Health Physics technician to perform the final survey.
The technician recorded his measurements on the shipping document and released the paperwork to the truck driver. The final review was not conducted. The' truck left the site at approximately 2100 hours0.0243 days <br />0.583 hours <br />0.00347 weeks <br />7.9905e-4 months <br /> on May 3, 1979, and traveled during the night hours.
It arrived at the Barnwell facility on the morning of May 4,1979. At 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />, the Health Physica foreman realized that a regulatory violation may have occurred. A Chem-Nuclear representative at Barnwell was contacted to verify the validity of the survey results. At 1150 hours0.0133 days <br />0.319 hours <br />0.0019 weeks <br />4.37575e-4 months <br />, the violation was confirmed by Chem-Nuclear. The South Carolina resident inspector for the Barnwell site.as informed of the.iolatica by Chem-Nuclear.
Corrective Actions:
1.
This incident will be reviewed with the Health Physics technicians involved and with the other RC&T technicians.
2.
The Health Physics radwaste shipment procedures will be revised with special highlighting of regulatory limits.
3.
An RC&T Foreman review will be required prior to future shipment releases.
