Atwood and Morrill Company, Inc.
Docket No. 99900049/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on March 21-23, 1979, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 and paragraph NCA-4134.5 of Section III to the ASME Code states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawing."

The Atwood and liorrill Procedure No. 20-67-01, effective date May 26, 1978, titled " Receiving Inspection by Quality Control,',' paragraph 4, states in part, " Obtains required specifications and drawings for material to be inspected...."
Shop Process Order 14008 for 16"-900#, Check Valve Cover,Section III, Class 1 (BM 40139, S/N 37-14008), Sequence 10, states, " Receiving inspection of plate to include the following:
(a) Visual inspection.
(b) Dimensional inspection."
AM Drawing. C-24257, Revision 1, titled " Cover 16" up to 900#" requires the outside diameter to be 29.75 inches + 0.120 inch (29.87" max and 29.'63" min).
Contrary to the above, although receiving inspection had processed the cover specified on Drawing C-24257 with reference to Process Order 14008, Sequence 10, the cover had an outside diameter of 29.4375 inches.

Paragraph 4.2.2 of Section 4 of the ASME accepted Quality Assurance Manual. Issue A, states in part, "... Work proceeds in accordance with the sequence of the operations listed in the Shop Process Order.
Operations are signed off and dated as completed by parties responsible, such as the Machine Operator, Quality Control Inspector... and, where required, by the Authorized Nuclear Inspector."
Contrary to the above, (a) Sequence 20, which signifies all welding and machining is complete, on Shop Process Order 14008, Item 75, (BM 40146, S/N 152-14008) 79060500q
for a 4"-1500#, Check Valve Body,Section III, Class 3, was signed off as complete on March 20, 1979, but the finished machining of the inlet weld end, had not been completed.
(b) Sequence 20 on Shop Process Order 13952, Item 05, (BM 40053, S/N 38, 39-13952) for 26"Section III, Class 1, Main Steam Isolation Valve Poppet, had been signed off as being domplete on March 22, 1979, however, weld numbers 1, 2, and 3, which are included in operation sequence No. 20, had not been completed.
