MAR 0 91979 APPEhTIX A NOTICE OF VIOLATION Babcock and Wilcox Company License No. SNM-778 Lynchburg Research Center Based on the hTC inspection January 31 through February 2,1979, certain of your activities were apparently not conducted in full compliance with h7C requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by License Condition No. 13, which incorporates Section 5,
" Administrative Control", of Appendix /., " License Conditions", of the licensee application dated May 20, 1970, the licensee establishes and maintains area operating procedures to coordinate and ensure the safe operation of activities at LRC. Hot Cell Procedure BW-lic-110, "Zircaloy Cutting Restrictions", requires that the operator calculate end record in the log book the quantity of zircaloy and plutonium fines generated and accumulated in the hot cells and that the supervisor initial the space in the log book showing that he had checked the calculations and agreed with the amounts.
Contrary to the above, the llot Cell No. 1 Log Book stowed no entries for the plutonium fines generated and accumulated, or the supervisor's initials.
This is a deficiency.
