Thank you for your letter of December 19, 1978, in response to our letter dated ! ovember 22, 1978. We have no further question con-cerning Item (1) and will review your corrective action during a future inspection.
Regarding Item (2), we find that additional information is needed.
With regard to Item (2), please provide the following information within twenty (20) days in order that we may complete our review:
1.
Your corrective action concerning the two specific cases identi-fied where calculational forms (for calculations E-0157-2-33-P and E-0478-965-3) were not completed and prepared in accordance with your procedure; your further actions to assure that these were the only incorrect calculational forms; and the date these corrective actions were or will be completed.
2.
Your specific preventative actions to assure this procedure will be followed in the future (e.g. ret. raining / indoctrination of user personnel and/or increased QA audit emphasis in this area) and the date those actions will be completed.
Should you have any questions concerning this matter, we will be pleased to discuss them with you.
