Westinghouse Electric Corporation Docket No. 99900005/78-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on Mcy 22-26, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
A.
Section 1 of the Westinghouse, Nuclear Fuel Division (NFD), Topical Report, WCAP-7800, Revision 4-A, states in part:
"The NFD Quality Assurance Program Plan complies with the AEC Quality Assurance Criteria, 10 CFR 50 Appendix B, and follows the guidance of the AEC Grey book, WASH 1283, Revision 1, May 24,1974, the Green book, WASH 1309, Revision 0, May 10,1974, and included and referenced regulatory guides and standards...."
The Grey book, includes and references regulatory guide 1.64 Revision 0, and ANSI, standard N45.2.ll, Draft 3, Revision 1, which standard states in part:
"4.2 Design Analyses.... Calculations shall be identifiable by subject (including structure, system, or component to which the calculation applies), originator, reviewer and date; or by other data such that the calculations are retrievable."
"6.2 Extent...
However, the applicability of standardized or previously proven designs, with respect to meeting pertinent design inputs, including environmental conditions, shall be verified for each application.... The original design and associated verifi-cation measures shall, however, be adequately documented and refer-enced in the files of subsequent application of the design."
Contrary to the above, the requirements of ANSI N45.2.ll were not fully implemented by Westinghouse NFD.
Examples are:
1)
Thermal and Hydraulic hand calculations THB 747 and records of checks of these calculations where not identifiable by subject, originator or date.
2)
Internal memo NE/NDI-918 relieved the requirement to perfonn design verification of reload nuclear designs when calculated values (design inputs) are within the Reload Safety Analysis Checklist parameters or are generated by now standard methods.
3)
The verification measures were not adequately documented and referenced in Thermal and Hydraulic Design Calculations THB 747.

Section 18 of Westinghouse,.'iFD, Topical Report, WCAP 7800, Revision 4-A, states in part:
" Audits of f1FD quality assurance related activities are conducted routinely under an audit program directed by the Product Assurance Department. The program is formalized with published procedures, check lists and schedules."
Westinghouse, Product Assurance Department I4anual, Section 2.18.1, Revision 1, states in part:
" Types of records that shall be maintained are... Checklist utilized during the audit.
Contrary to the above -~ecords one of the three Product Assurance Department audits :eviewed did not have the checklist utilized during the audit.
