License No. 40-18164-01 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on September 13, 1979, certain activities appear to be in noncompliance with NRC regulations and the conditions of your License No. 40-181F4-01 as indicated below:
1.
License Condition 13.A(1) requires that sealed sources shall be tested for leakage and/or contamination within six months prior to being put into use.
Contrary to this requirement, a one curie cesium-137 source contained in a Kay Ray Model 7064 source holder was not tested six months prior to initial installation and use during July 1978.
This is an infraction.
2.
Contrary to 10 CFR 19.11(a) or (b), neither the documents required by 10CFR 19.11(a) nor the notice required by 10 CFR 19.11(b) were posted at the time of the inspection.
This is a deficiency.
