OCT 2 41919 APPENDIX A NOTICE OF VIOLATION University of Virginia License No. R-66 University of Virginia Test Reactor Facility Based on the NRC inspection September 18-21, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indi-cated below.

These items have been categorized as described in correspondence to you dated December 31, 1974.

As required by 10 CFR 20.203(c)(2), each entrance to a high radiation area shall be:

(i) equipped with a control device to reduce radiation levels below 100 arem/hr; or, (ii) equipped with an alarm; or, (iii) maintained locked; or, (iv) under direct surveillance.

10 CFR 20.202(b)(3) defines a high radiation area as an area, accessible to personnel, with radiation levels such that a major portion of the body could receive in any one hour a dose in excess of 100 mrem.

Contrary to the above, from March 9 to 30, 1979 and July 6, 1979 radiation levels in excess of 1000 mrem /hr existed in the demineralizer room without any of the regi. ired controls. Specifically, the installed alarm deactivated when the reactor was shutdown, the door was not locked, and surveillance was not provided.

This is an infraction.
