99900036/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 13-17, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR 50 states:

" Activities affecting, quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate t-circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, precedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

A.

Paragraph 2.3.1 in QCP 9.2, Revision C, states in part, " Manufacturing is responsible for manual and automatic welding to be performed within the welding parameters as specified in the applicable DWP.... The method of supervising that manual welding is being performed within the specified welding parameters is outlined as follows:

. The foreman shall attach a ac,te to the machine if the indicator registers outside the welding parameters. The note shall cite the date, name of the fore-man, machine numbec and actual amperage reading.

" Paragraph 2.1.7 in QCP 9.2, Revision C, states in part, "All material requisition cards shall be taken by the welder to the weld material storage area (rod room) and given to the attendant for filling of the order and also for veri-fication that all of the information on the requisition card is correct and complete. Before the welder is given the materials, he shall initial the original copy of the requisition card in space 13, certifying that he has read the DWP and fully understands the contained instructions and restrictions.

Contrary to the above:

Welding of tie straps on Contract No. 12473, Job and Control Number 74917-005, was observed being performed in the 2G position with 3/16 inch E 308 electrodes, despite the applicable welding procedure, DWP MA-801-0, allowing only the 1G position to be used with this size of electrode and the welder having initialed the original copy of the material requisition card to certify he had read the DWP and fully understood the contained instructions and restrictions.

The note attached to machine S/N 2615, which was used for performing the welding operation described in 1. above, did not cite the date, name of the foreman, machine number or actual amperage reading. This machine was verified for the inspector to be producing an actual am-perage value of 175 from an indicator setting of 300 amperes-1 2166 012 7 nacio D31

EN 82 gas tungsten arc welding vire was issued for use on Contract No. 12373, Job and Control No. 748171-085, without the assigned welder initialing the original copy of the requisition to certify he had read and understood the DWP.

Similarly, Linde 124 flux was issued for use on Contract No. 72273, Job and Control No. 725256-040, with only one of the two assigned welding operators initialing the original copy of the requisition.

B.

Strip cladding operations on primary piping were required by operation C7'i on the traveler for Contract No. 73474, Job and Control No.

766722-044, to be performed in accordance with Detail Welding Procedul e Specification (DWPS) SAA-1.8-105-1.

Shielded metal arc welding of whip restraint lug seams was required by Operation 0743 on the traveler for Contract No. 74474, Job and Control No. 767741-017, to be performed in accordance with DWPS SHA-1.3-109-1.

Contrary to the above, velding operations on the referenced items were observed, which were not in accordance with certain requirements of the applicable DWPS.

C.

Paragraph NB-4231.2 in Section III of the ASME Code states ia cart,

" Attachments which are welded to the component during constr. cion but which are not incorporated into the final component, such as alignment lugs or straps, tie straps... are permitted, provided.

(a) The caterial is identified and is suitable for welding but need not be certified material...."

Contrary to the above, unidentified alignment and tie strap materials were observed welded to:

1.

Lower vessel assembly - Contract No. 75174, Job and Control No.

768141-006.

2.

Piping subassembly - Contract No. 74474, Job and Control No.

767741-019.

3.

Two piping subassemblies - Contract No. 74474, Job and Control Nos. 767711-021 and 767711-022, i.e.

strapping connecting the subassemblies for rotation in submerged arc wu: ding.

D.

Systems No. 6 of the Quality Assurance Manual, paragraph 2.3.2.2.5, states in part, "As procedures are modified, adJed or deleted... drawings reflecting applicable M&P specifications shall be updated and reissued by Design Engineering.
