Appendix A NOTICE OF VIOLATION Detroit Edison Company Docket No. 50-341 Based on the results of the NRC investigation conducted on January 3 and 4, 1979, it appears that certain of your activities were not con-ducted in full compliance with NRC requirements as noted below.
Item 1 is an infraction and item 2 is a deficiency.
1.
10 CFR Part 50, Appendix B, Criteria IX, states in part that,
" Measures shall be established to assure that special processes, including welding,
. and nondestructive testing are con-trolled and accomplished by qualified personnel..." The Enrico Fermi Atomic Power Plant Unit 2, PSAR Section I-D.l.51.
commits to 10 CFR Part 50, Appendix B, and the General Electric Installation and Service Engineering Division, NDE Personnel Qualification and Certification Procedure, NDE-PQ-8000, Rev. 5, 3/10/78 and SNT-TC-IA requires that all levels of NDE personnel be re-certified at least once every three years. Materials Assurance Company, Qualification and Certification of Nonde-structive Examination Personnel Procedure No. MAC-77-026, Rev. 4, 12/28/77 and SNT-TC-IA r2 quires the following:
a three month work timc experience prior to taking the Level Il liquid penetrant test.
a high school graduate requires 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> of training prict to the test.
a general, specific and practical test is required as the test and a composite grade of 80 or greater is required for qualification.
an annual eye test by a qualified medical professional is required.
all certification shall be automatically terminated when an employee leaves the employer where he had been certified.

Appendix A Contrary to the above, measures were not sufficient to a.
assure that welding and nondestructive testing activities were accomplished by qualified personnel.
Examples of improper qualification are: a (CE) individual had con-tinued performing liquid penetrant examinations in sccor-dance with ASME Section III 6 V on safety related wilds after his liquid penetrant certification had expiret, b.
A Materials Assurance Company individual who had not had the experience, training, or an eye test as shown on a certification record performed liquid penetrant examinations in accordance with ASME Section III and V on safety related welds.

10 CFR Part 50 Appendix B, Criteria XVII, states in part that,
" Sufficient records shall be maintained to furnish evidence of activities affecting quality.
Records shall be identifiable and retrievable.
." The Enrico Fermi Atomic Power Plant Unit 2 PSAR Section I-D.l.5p commits to 10 CFR Part 50, Appendix B.
Also Materials Assurance Company's Qualification and Certification of Nondestructive Examinations Personnel, Procedure No. MAC-77-026, Rev. 4, 12/28/71 states in part that, " Certification records shall be kept on file in Materials Assurance Company office and maintained by the NDE Specialist. The certification records shall consist of the following:
. copies of current examinations and of grades for all previous examinations and descriptions of practical test objects.
Contrary to the above, the Materials Assurance Company NDE Specialist did not maintain records of an individual's test to confirm that the individual did take the test in liquid penetrant examination and that he was certified, g.
