APPENDIX A NOTICE OF VIOLATION Combustion Engineering, Incorporated Docket No. 70-1100 Power Systems 1000 Prospect Hill Road Windsor, Connecticut 06905 License No. SNM-1067 Based on the results of an NRC inspection conducted on November 1-3, 1978, it appears that one of your activities was not conducted in full compliance with the conditions of your license as specified below.
This is an Infraction.
Paragraph 14.7 of the license application, incorporated as Condition 9 of Facility License No. SNM-1067, requires, in part, that the adequacy of (ventilation) sampling techniques to obtain represe7tative samples be verified.
Contrary to the above, the adequacy of ventilation sampling techniques to obtain representative samples was not verified.
While seven " isokinetic" probes designed in accordanca with ANSI N13.1-1969 were recently installed it was not verified that these probes, as installed and operated, result in representative sampling of the ventilation system.
Three additional probes, installed as original equipment and not replaced by ones of modified design, have also not been evaluated to verify that their design and operation result in representative sampling of the ventilation paths in which they are installed.
