Or November 3, 1978, in accordance with 10CFR50.55(e), we noti-fied Mr. A. Toth of the NRC that a deficiency existed in Bailey Controls power supplies installed in ten Category I applications, principally HVAC analog instrumentation control loops.

This letter serves as our 30-day written report for this deficiency.

DESCRIPTION OF THE DEFICIENCY Bailey Controls was advised by their power supply vendor (ITT North Electric) that random premature failures of the power sup-plies could occur due to contamination of a capacitor by the packaging material used by the vendor.

The packaging material of concern is foamed-in-place urethane which may damage the capacitor's dielectric material.

A review of the part numbers supplied by Bailey (BCC Part Nos.

8080B02P008 through 8080B02P0012, inclusive) determined that such power supplies were installed in ten Category I applica-tions.

These applications are primarily HVAC analog instru-mentation control loops such as RBSBV, CRAC, etc.

These systems are important for post LOCA and off-site release control.

CORRECTIVE ACTION All potentially defective power supplies will be replaced.

Bailey Controls has notified us that replacement power supplies will be shipped to us in early December 1978.

are fully aware of this problem and will institute corrective action to insure that the nackaging material utilized in the future willnot adversely a'ffect component reliability.

DATE OF FULL COMPLIANCE We estimate that all replacements of the affected power sup-plies will be completed by April 1, 1979.
