i Appendix A NOTICE OF VIOL \\ TION St. Mary's College License No. 22-00027-06 Based on the inspection conducted on May 28, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Item I is an infraction, and item 2 is a deficiency.
1.
License Condition 14 requires leak tests of certain sealed sources in your possession at six month intervals for gamma and beta emitting sources and three month intervals for alpha emitting sources.
Contrary to the above, from October, 1979 to the date of this inspec-tion, no leak tests had been performed on the 40 millicurie Cs-137 sealed source, the plutonium-berylium sealed source, the 99 micro-curie Pu-238 source and the 25 millicurie cd-109 sealed source.
This is a repeat item of noncompliance.

License Condition 15 states that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures listed in letter dated January 24, 1975.
Item 3 of that letter states that surveys would be taken monthly of areas where unsealed sources were in use and that the results of these surveys would be recorded.
Contrary to the above, from October, 1979 to the date of this in-spection, records of the results of these surveys had not been maintained. This is a repeat item of noncompliance.
