General Atomic Company Docket Number 99900265/78-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on November 27-December 1,1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion XI of Appendix B to 10 CFR 50 states in part, "All testing required to demonstrate that... components will perform satisfactorily
.. is identified and performed in accordance with written test proce-dures."
Article NCA 4134.11(b) of Section III of the ASME Code states in part,
" Test procedures shall include provision for assuring... that adequate instrumentation is available and used." In addition, the QA Manual, Procedure No. 11, Revision G, and QDI-HT-3801, Issue B, titled Hydrostatic Testing of Pressure Vessels, paragraph 4.0, subparagrah 4.2 states, "The test shall be accomplished at a temperature that will minimize the possibility of brittle fracture. The test pressure shall not be applied until the component, appurtenance or system and the pressurizing medium are approximately the same temperature."
Contrary to the above requirements, during a low temperature hydrostatic test of two (2) pressure vessels, temperature measuring equipment had not been provided for monitoring the temperature of the component, and the pressurizing medium.
