The unit was shut down January 12, at 2322,foi refueling.

Correctiva action:

2B-N condenser was leak checked. Four tubes were protruding out the tube sheet; these were re-rolled. Three tubes were plugged that indicated leaks. The i

condenser was returned to service.

3 N/A CD VALVEX from the grid because of an APRM high flux scram. This scram was caused by a pressure spike which resulted from MSIV A failing closed due to an appar-ent stem-disc separation.

Corrective Action: Returned unit to se vice with

'A' main stean line out or' service. The MSIV's will be inspected and repaired during the upcoming refueling outage.
