NOV 2 81979 APPENDIX A NOTICE OF VIOLATION University of Virginia License No. R-66 Based on the results of the NRC inspection conducted on October 17-20, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below.

These items have been categorized as described in our correspondence to you dated December 31, 1974.

A.

Technical Specification 4.4 states radiation monitoring equipment required by Technical Specification 3.2 shall be calibrated at intervals not to exceed sight months. Technical Specification 1.11 defines a channel calibration as an " adjustment of the channel such that its output responds, with acceptable range and accuracy to known values of the parameter which the cannel measures."

Contrary to the above, as of October 20, 1978, calibrations of the Reactor Room Constant Air Monitor, required by Technical Specifi-cation 3.2, did not include calibration to reference values of the parameter the channel measures (i.e., a reference radioactive source).

This is an infraction.

B.

10 CFR 20.401(b) requires the licensee maintain records showing the results of surveys required by 20.201(b).

Contrary to the above, records of weekly radiation surveys, docu-menting compliance with gosting requirements of 10 CFR 20.203, were not complete for the period January thru July 1978.

Specifically, records of 10 weekly surveys in this period were not maintained.

This is a deficiency.
