REBAR BENDING NOT IN ACCORDANCE WITH SITE PROCEDURES Description of Deficiency This audit report identifies a problem in which activities involving bending of rebar was not in accordance with recently revised construction engineering procedures (CEP's). The revisions were to bring Hartsville CEP's into compliance with General Construction Specification G-2, Revision 2. The subject CEP revision required design approval of bending of partially embedded rebar before bending. At the time of the audit, the new revision was not being followed by the site.

Safety Implications Statement The subject revision to the G-2 Construction Specification was to bring G-2 into compliance with the ACI 318-1971 building code which requires design approval before the bending or straightening of partially embedded rebar was performed. Before the revision of the G-2 specificatien, other codes and procedures were applicable to provide construction personnel with the guidance to ensure any bending of rebar was done properly and without damage to the rebar. At the time these procedures were written, prior design approval of rebar bending was not required by G-2 and was an acceptable means to bend rebar. In addition, each site had a quality control program which ensured the requirements of these procedures were correctly applied. Therefore, partially embedded rebar which was bent in accordance with these procedures af ter the G-2 revision are acceptable as is, because the quality of bent rebar was not altered by the revision. Thus, the deficiency does not represent a safety hazard.

Corrective Action The affected construction personnel have been notified of the revision to G-2 and the CEP's are now requesting design approval before bending of partially embedded rebar. For STRIDE designs, STRIDE Specification 300-41 is being revised to require approval of GE/C. F. Braun before bending of partially embedded rebar.
